The purpose of this form is to report issues or situations that might be violations of Bear Lake West Declaration of Restrictions (sometimes called CC&R's).  Please be as specific as you can stating what you think the violation is and lot # or address.  You can view a copy of the neighborhood plat map in you look in the "Documents" tab.
